Â鶹tv¹ÙÍø and the Cooper family have always had a dedication to giving back to their local communities. If you would like to request a meat or monetary donation please complete all areas of this form. Any large monetary requests will go through the Cooper Family Foundation and will have a longer processing time. If a request is time sensitive, be sure to list the deadline in the form. Please note that Â鶹tv¹ÙÍø strives to primarily meet the needs of our local communities in Ohio and Indiana.
